DEPARTMENT FUNCTION/JOB OVERVIEW

Responsible for planning and implementing daily recreational, social and therapeutic activities for patients/residents in long term care program.  Responsible for the assessment, development, and implementation of a recreation care plan for each patient/resident.  Responsible for establishing an environment that empowers patients/residents and utilizes patientâ??s/residentâ??s interests and preferences to motivate and maximize personal health. 

Functions as part of an interdisciplinary treatment team.  Functions as a member of facility wide committees. Delegates assignments to volunteers and student interns. Work will be performed in an ethical and legal manner following organizational policies, processes, protocols and procedures. Responsible for quality service delivery and internal/external customer relations for Madonna as a whole, including upholding the mission and values for the department and facility.

ESSENTIAL FUNCTIONS

  1. Compiles, analyzes, and copies (records and documents) information obtained from patient/resident evaluations and coordinates each patientâ??s/residentâ??s activity pursuit plan with their comprehensive care plan, including:
  2. Completing Resident Assessment Inventory (MDS,  CAAâ??s, etc.)
  3. Documenting and updating residentsâ?? interventions on care plan.
  4. Documenting progress towards goals.
  5. Completing a Leisure Pursuit Assessment.
  6. Plans and coordinates daily the resources necessary to meet the recreational and social needs of patients/residents.  This includes planning and implementing daily groups, individual treatment sessions, outings and other services for long term care patients/residents.  Contributes ideas for planning activities on a monthly basis.
  7. Compiles, copies and maintains (records and documents) statistics for the recreation program, including patient/resident participation in group and individual treatment sessions, and monthly documentation (or as needed) reflecting progress towards patient/resident goals.
  8. Analyzes statistics to ensure recreation programming is meeting the needs of the patient/resident. Makes changes to patient/resident care plans as needed.
  9. Conducts interviews (verbal or written) with patients to obtain leisure history; obtains patientâ??s/residentsâ?? current and past leisure preferences and lifestyle.
  10. Leads, supervises, instructs and assists patients/residents in groups, individual treatment sessions, and community outings on a daily basis in order to meet the needs of the patients/residents.
  11. Participates as a member of the treatment team in order to determine and meet the needs of the patients/residents.
  12. Delegates assignments and supervises, volunteers, and students to carry out recreation and social activities, according to the patientâ??s/residentâ??s care plan.
  13. Communicates,(speaks-signals) with supervisor, co-workers, and other staff regarding developments, concerns, goals, issues related to job duties and responsibilities.
  14. Serves on department and facility wide committees and groups as assigned by supervisor.
  15. Must perceive customer relations as a primary focus, and continually strive to satisfy the needs of every customer in all areas of the work force.
  16. Follows instruction from supervisor to perform other functions as assigned in order to achieve the goals within the department.

  18. necessary equipment and performs required duties according to established safety standards to maintain compliance with regulations and prevent injury.
  19. Responsible for maintaining a clean and clutter free environment in the Recreation/Dining area and lounges on the patient/resident care units.
  20. Responsible for identifying, maintaining, and organizing supplies and equipment needed to carry out recreation programs.


    PHYSICAL DEMANDS AND ENVIRONMENTAL CONDITIONS

    Medium work with frequent lifting.  Manual patient transfer or reposition which does not exceed the equivalent of 35 pounds of force to lift, push or pull. Requires sufficient strength to produce  35 pounds of force to push a wheelchair with a 300-pound individual on level surfaces. Constant walking with maximum duration of up 30 minutes sustained walking on tile, carpet and other surfaces. Must tolerate frequent standing with sustained periods of up to 1 hour on tile, carpet, and shower surfaces. Must tolerate frequent sitting with sustained periods of up to 1 hour.  Frequent reaching with both arms at low, waist, and high levels. Frequent grasping of both hands for transfers and repositioning requiring normal grip strength based on national averages. Frequent stooping, bending, and twisting are required for patient/resident care. Constant near visual acuity to evaluate and monitor patients/residents.  Ordinary hearing and speaking skills.  May be exposed to biohazardous materials. Mobility required for travel to scheduled events or meetings as needed.

               
    QUALIFICATIONS (Education/training and/or Experience)
  • Bachelors degree and a minimum of 1 year experience planning, conducting, and supervising activities in a long term, geriatric, or institutional setting required.
  • Bachelorâ??s degree in health and human service field preferable, however, additional applicable work experience may be considered in lieu of formal education.
  • Must have completed (or will complete within 1 year from date of hire) NHCA Training Course for Activity Directors or comparable course.
  • Must have valid Nebraska driverâ??s license and proof of good driving record.
  • Requires CPR for Health Care Provider/CPR for Professional Rescuer.
