Position Summary | The Medical Case Manager will provide various levels of case management services to individuals living with HIV/AIDS including assessment, referral and support for participants of Ryan White grants. This position will assist patients enrolled in Ryan White services as well as assist with housing instability, food insecurity, mental illness and other issues through a referral network. This position will assist patients with Ryan White enrollment and renewals including State, Federal and community programs. This individual will support the Ryan White team and clinical staff. |
